A: When complex care episodes such as surgical procedures do not consider
the patients' bigger pictures or adequately manage them across the care
continuum, the result is a fragmented experience, strain on healthcare
providers and staff, poor outcomes, and higher than necessary costs. We
help bring those episodes of care into sharp focus. Informal
caregivers--family or friends--play the most significant role,
quantitatively and qualitatively, in coordinating a patient’s care. But
even with today’s coordination and patient engagement solutions, they’re
the least equipped to play that role. We put the informal caregiver
front and center, providing them with the tools they need.
