ClinicalBox, Inc. creates software solutions that are mission critical for healthcare providers who serve complex, high-cost patients requiring coordination and engagement: the 1% of patients driving 20% of healthcare costs. ClinicalBox does this by treating all interactions with the healthcare system as episodes of care. For each episode of care there is a template. And associated with these templates are milestones, tasks, interactive educational modules, communication and notifications, which are cleanly nested within an intuitive timeline. ClinicalBox solutions are for patients, their families and their healthcare providers all of whom can leverage its capabilities to get on the same page. ClinicalBox is capable of integrating with all the major EHR systems. ClinicalBox solutions enable delivery systems to seamlessly execute value-based risk-sharing contracts with health insurers. The ClinicalBox team brings significant experience to the table: a combined 75 plus years of healthcare and engineering experience; 24 start-ups between the members of the leadership team; and $1.97B in M&A and private equity transactions.

Q: Please tell us how your business idea was conceived. Was there an "aha" moment or did it evolve gradually?

A: Our business idea evolved gradually over eight years before we decided that we either needed to stop complaining about problems in the healthcare system or to do something about them. We spent a couple years working on our ideas on weekends, talking with customers, and narrowing our focus. On one very long road trip it all came together: Coordination and patient engagement is where healthcare is headed. We realized that we want to help make waves and then ride them as the healthcare system moves towards its value-based future.

Q: What's the most inventive, innovative, or disruptive aspect of your initiative?

A: We design for the 1 percent of patients who drive 20 percent of healthcare costs, and for use by the most technophobic patients and healthcare providers. We are disruptive by virtue of offering brutally simple solutions to the everyday challenges of navigating and managing complex episodes of care; by defining "patient-centered" to include the patient's family; and by doing away with expensive up-front implementation costs to price on a per-patient basis for the duration of the episode of care. Our mantra is: You use it, you get value, and then we get paid.

Q: How will it help people live to their greatest potential or contribute to making the world healthier?

A: When complex care episodes such as surgical procedures do not consider the patients' bigger pictures or adequately manage them across the care continuum, the result is a fragmented experience, strain on healthcare providers and staff, poor outcomes, and higher than necessary costs. We help bring those episodes of care into sharp focus. Informal caregivers--family or friends--play the most significant role, quantitatively and qualitatively, in coordinating a patient’s care. But even with today’s coordination and patient engagement solutions, they’re the least equipped to play that role. We put the informal caregiver front and center, providing them with the tools they need.

Q: Five years from now, what would you like to be able to say has been your most important contribution to health?

A: Our five-year goal is to create solutions that help patients--specifically the 1% of people at the fringes of our healthcare system who are suffering and looking to the healthcare system to become well--their family members, and their healthcare providers. We aim to do that without adding hassle, clicks, time, or confusion, and we'd like to be able to say that our product did what it was supposed to-- coordinated care and engaged patients--and led to a healthcare system that makes people as whole as it possibly can.

A: Raw unbridled passion for problem solving.

Dr. Salim Afshar

Dr. Afshar knows care coordination intimately. He worked as a care coordinator after college, followed by receiving his MD and DMD from Harvard, completed his residency within the Partners Healthcare System, Chief Residency at Massachusetts General Hospital (MGH) and in total having experience at 17 healthcare facilities to date. Dr. Afshar receives regular inspiration as a practicing surgeon at Boston Children's Hospital. During the past 15 years he has solved system and design challenges both large and small with solutions that were inspired by processes and systems found in nature (biomimicry). He played a key advisory role in the implementation and design of healthcare software systems over the past 10 years. In addition to his work at ClinicalBox and Boston Children’s Hospital he is an expert-in-residence and start-up mentor at the Harvard Innovation Lab.

Dr. Hagigi has lived and breathed coordination, patient-engagement and value-based healthcare for the past 13 years. He brings his significant experience across the healthcare industry to ClinicalBox's software solutions. Prior to ClinicalBox he was a management consultant at The Boston Consulting Group (BCG) where he collaborated with C-suite executives on significant strategic and operational challenges facing their businesses. At BCG he was a core member of healthcare and technology practice areas. And prior to BCG he advised healthcare organizations (e.g., healthcare delivery systems, biotech, global health) on improving coordination and performance. In addition to being the CEO of ClinicalBox he is a Visiting Scholar at Brandeis University with a focus on care coordination research, an expert-in-residence and start-up mentor at the Harvard Innovation Lab, and an Easton Technology Leadership program mentor to MBA students at UCLA.
